When applying for the J. Armand Bombardier Scholarship Program:

· Identity (last name, first name)

· E-mail address

· Mailing address

· Telephone number

· Academic information

· Amount shown on line 199 of your Québec income tax return (yours, your parents' or your spouse's, as applicable)

· If your application is accepted :

o Banking information

o Social insurance number

What for?

· To process the scholarship application

· To deposit the scholarship into your account

· To produce the T4 form

Activity 3 – Application for the Yvonne L. Bombardier scholarship in visual arts

When you apply for the Yvonne L. Bombardier scholarship in visual arts:
